Typeform vs Jotform vs Formsort vs PathwAI for healthcare intake
Typeform, Jotform, Formsort and PathwAI compared for healthcare intake: HIPAA and BAA plans, routing, payments, ad attribution and published pricing.
Written for operators, growth leads and product managers at telehealth and DTC health brands
Pick by what the intake has to do. For a simple HIPAA form on a small budget, Jotform on its Gold plan is the shortest path. For a polished one-question-at-a-time form where a BAA is a contract conversation, Typeform works on Enterprise. For a product team with engineers that wants deep control over a flow, Formsort fits. For a telehealth or DTC health brand that needs eligibility routing, consent, checkout and ad attribution in one flow, PathwAI is built for that.
Verdict by use case
- Telehealth or DTC brand acquiring patients from ads: PathwAI. Eligibility, intake, consent and checkout on one canvas, with server-side conversions to ad platforms.
- Product team with engineers who want to build more themselves: Formsort.
- Clinic or small practice that needs HIPAA forms quickly: Jotform on Gold.
- Brand already on Typeform for marketing, now adding patient data: Typeform, but only on a plan that includes a BAA.
What is the difference between a form builder and an intake funnel builder?
A form builder collects answers and sends them somewhere. Typeform and Jotform are general form builders that serve every industry, with healthcare as one use case.
An intake funnel builder treats the form as the front door of a patient journey. It decides who is eligible, collects consent as a record, takes payment, and tells your ad platforms which campaign produced the patient, without sending health data to them. Formsort sits between the two: a flexible flow builder with healthcare as a main focus. PathwAI is built as an intake funnel builder for health brands.
The difference matters most after launch. A form builder answers “did they submit?” A funnel builder answers “which step lost them, and which ad brought the ones who paid?”

PathwAI
Best for: telehealth and DTC health brands that acquire patients online and need intake, checkout and attribution in one flow.
PathwAI is an AI-native builder for patient intake flows, eligibility quizzes and checkout funnels. Eligibility, intake, consent and checkout steps sit on one visual canvas in the flow builder. Routing rules branch on any answer or combination of answers: eligible patients go on to intake and checkout, ineligible patients get a custom message, a waitlist or another treatment path. Your clinicians set the criteria; the builder carries them out.
Strengths:
- Consent is its own step. Patients must acknowledge before continuing, and the consent is timestamped and stored with the intake data.
- Steps can take payments (Stripe, Authorize.Net), book appointments (Cal.com) and verify ID (Persona).
- The Data Platform sends conversions server-side to Meta, Google Ads, TikTok and Microsoft Ads. Email and phone are SHA-256 hashed, PHI such as date of birth and condition is stripped, and only visitors who consented are sent. No third-party ad pixels.
- Step-level completion and drop-off for every screen, plus A/B tests run by non-technical teams.
Limitations: no public price, so you have to talk to sales. The BAA is on the Enterprise plan only. PathwAI is not SOC 2 certified at this time. It is not an EHR, so records still live in your EHR or care platform. If all you need is a contact form, it is more than you need.
Routing on the PathwAI canvas: every path from one question to its outcomes.
Typeform
Best for: teams that want a polished, one-question-at-a-time form, often already using Typeform for marketing.
Typeform is a general form builder known for its conversational layout. Its pricing page lists a free plan and paid plans from Basic ($25/month billed yearly) up to Business ($83/month billed yearly), with Talent, Growth Flow and Enterprise above that. Logic to “connect, skip, and rearrange questions” and Stripe payment questions are part of the product.
Strengths:
- A conversational, one-question-at-a-time layout.
- Integrations include Google Tag Manager, Google Analytics and the Meta pixel, for marketing teams.
- Typeform’s Enterprise page lists SOC 2 and HIPAA compliance, and US or EU data centers.
Limitations: HIPAA is not on the self-serve plans. Typeform’s pricing page marks HIPAA compliance as Enterprise only, and its help center says a BAA is available to Enterprise and Growth Custom customers, requested through sales. Both are custom-priced. And browser pixels are a poor fit for patient pages: HHS warns that regulated entities may not use tracking technologies in a way that discloses PHI to tracking vendors.

Jotform
Best for: clinics, practices and small teams that need HIPAA forms quickly and cheaply.
Jotform is a general form builder with ready-made templates, including HIPAA-enabled ones. Jotform says that “by signing up for either the Gold or Enterprise plan,” you get HIPAA-enabled templates, integrations and plug-ins, and that covered entities who enable the HIPAA features receive a signed BAA. Gold is $129/month, or $99/month billed yearly, with 100 forms and 10,000 submissions a month. Starter, Bronze and Silver do not include HIPAA.
Strengths:
- The lowest published price for a plan with a BAA of the four tools here.
- Accepts payments through Square, PayPal, Stripe, BlueSnap, Authorize.Net and others, per its HIPAA FAQ.
- Jotform says it is PCI DSS Service Provider Level 1 compliant.
- Conditional logic can show or hide fields, skip to pages and show different thank-you messages.
Limitations: it is a form, not a funnel. Logic and payment fit in one form, but multi-path journeys, step-level drop-off and ad attribution are up to you. Jotform’s HIPAA FAQ notes that integrations on HIPAA forms are limited to services that can also support HIPAA, and that submission data is not sent by email: you log in to view it.

Formsort
Best for: product and engineering teams at health companies that want detailed control over a flow.
Formsort describes itself as a tool for product teams to build “onboarding, check-ins, quizzes and more,” and says it was “built from the ground up with healthcare companies in mind.” Its pricing page lists a Free plan (100 flow starts a month), Pro at $279/month (1,000 flow starts) and Enterprise by contact. HIPAA compliance is on Enterprise, and Formsort says it “will sign a BAA with enterprise clients.” Its security page says it is SOC 2 Type II certified.
Strengths:
- Deep design control and data destinations: Stripe, Segment, Google Analytics, Amplitude, BigQuery, PostgreSQL, Redshift, Salesforce.
- Analytics events per step (StepLoaded, StepCompleted, FlowFinalized) sent through Google Tag Manager or analytics tools.
- Formsort says it stores data only briefly in transit to its destination.
Limitations: the flexibility pays off most with technical help on hand, because analytics and ad tags are wired through your own stack. HIPAA requires the Enterprise plan, which has no public price. We did not find server-side, hashed ad conversions as a built-in feature on its site.

Head to head
Typeform vs Jotform
Typeform’s strength is the respondent experience. Jotform’s is price for HIPAA: its BAA comes with a published plan at $129/month, while Typeform’s BAA needs a custom-priced Enterprise or Growth Custom contract. Jotform also lists more payment processors. If your patients are on phones and the form is short, Typeform’s layout suits it. If a signed BAA this week matters more than looks, Jotform is the faster route.
Jotform vs Formsort
Both offer a BAA, but on different terms. Jotform’s is on a self-serve plan; Formsort’s is on Enterprise, priced by quote. Formsort gives a product team more control over design, step analytics and where data goes. Jotform asks less of your team to get started. Choose Formsort if you have engineers and a roadmap; choose Jotform if you need forms live this week.
Form builder vs intake funnel builder
For a telehealth or DTC brand, the form is the start of an acquisition funnel. That brings three questions a form builder does not answer for you:
- Routing. Can an ineligible patient get a different path, not just a different thank-you message?
- Consent and checkout. Is consent stored as a timestamped record, and does payment happen in the same flow?
- Attribution. Can you tell Meta or Google which campaign produced a paying patient, without a third-party pixel on a page that holds health data?
If all three matter, you need a funnel builder. If none do, a form builder is cheaper and simpler.
When PathwAI is the better choice, and when it is not
PathwAI is the better choice when:
- You run a telehealth or DTC program (for example GLP-1, weight loss or HRT) and patients arrive from paid ads.
- Eligibility decides the path, and ineligible patients need a real alternative.
- Your growth team needs conversions in Meta and Google Ads without sending PHI to them. The Data Platform hashes identifiers, strips PHI and only sends visitors who consented.
- Non-technical operators need to change copy, step order or routing and A/B test it without engineering.
PathwAI is not the right choice when:
- You need a single HIPAA form with a published price. Jotform Gold is simpler.
- Your team wants to engineer every detail of a flow in its own stack. Formsort was built for that.
- You need a vendor with a SOC 2 report today. PathwAI is not SOC 2 certified at this time.
- You need an EHR or practice management system. PathwAI connects to tools such as Healthie and CarePortals but does not replace them.
How it works in PathwAI
- Describe the program in plain English and the AI agent drafts a flow: steps, copy and routing. Your team reviews it; nothing publishes without approval, and consent wording goes to your clinical or legal team.
- Adjust eligibility rules and branches on the visual canvas. Add consent, payment, booking or ID verification steps.
- Publish on your own domain or as an embed.
- Turn on the Data Platform so completions and purchases reach your ad accounts server-side.
- Watch step-level drop-off in analytics and A/B testing, and test changes against a control.

Checklist before you choose
- Which plan includes the BAA, and what does that plan cost per year?
- Who signs the BAA, and how long does it take?
- Which integrations stay available once HIPAA features are on?
- Can ineligible patients take a different path, or only see a different message?
- Is consent stored as a timestamped record with the answers?
- Can the flow take payment, book a visit or verify ID in the same session?
- How do conversions reach your ad platforms, and does any health data go with them? Read the HHS guidance on business associates and the FTC’s Health Breach Notification Rule if you are not a covered entity.
- Can a non-technical teammate change the flow without a ticket?
Frequently asked questions
Is Typeform HIPAA compliant?
Typeform’s pricing page lists HIPAA compliance on the Enterprise plan only, and its help center says a BAA is available to Enterprise and Growth Custom customers through sales. The lower plans are not set up for PHI.
Which plan does Jotform need for HIPAA?
Gold or Enterprise. Jotform says covered entities that enable its HIPAA features receive a signed BAA. Gold is $129/month, or $99/month billed yearly.
Does Formsort sign a BAA?
Yes, on Enterprise. Formsort says it “will sign a BAA with enterprise clients” and that it is SOC 2 Type II certified. Enterprise pricing is by quote.
Can I put a Meta pixel on my intake form?
Be careful. HHS guidance says regulated entities may not use tracking technologies in a way that discloses PHI to tracking vendors. A safer pattern is to send conversions server-side, with hashed identifiers and only for visitors who consented, which is how PathwAI’s Data Platform works.
Is PathwAI cheaper than Jotform?
PathwAI has no public list price; it is scoped to your volume, integrations and compliance needs, and usually starts with a short pilot. If you only need a HIPAA form, Jotform Gold will likely cost less. Ask for a quote on the pricing page.
